The Physician Graduate license is a limited license which can be issued to an applicant who has graduated from medical school and is not currently enrolled in a board-approved postgraduate residency program. Physician Graduates must enter into a supervising agreement with a Texas licensed sponsoring physician prior to the issuance of the limited license.

 

To activate your license for the first time, you need to pay your first time registration fee within 90 days of your license issue date. To maintain an active license, you will need to renew your registration every two years.

 

Physician Graduates can register online up to 60-90 days prior to expiration. Registration reminders will be sent out at least 60 days in advance of the expiration date.  These reminders will include the “Sponsoring Physician Attestation” form.

There are five steps to submit your online license renewal:

 

  1. Go to the Physician Online Registration/Renewal system. Please note that if you are not able to complete your online license renewal, your data will be automatically saved for you for 15 days. Upon your return to complete the renewal, you will be given the option to use your saved application or to begin again.
  2. Enter your License Number and your Social Security Number.
  3. Review, confirm or update your information, and complete your registration.
  4. Pay the registration fees with MasterCard, Visa, Discover, American Express, or Electronic Check.
  5. View and print receipt.

 

Note: The “Sponsoring Physician Attestation” form must be completed by your sponsoring physician/physicians prior to your renewal. Once this completed form is received and processed, you will be able to complete your renewal.

 

The Physician Specialty List should be used with application and registration forms.

Once the Board has issued you an official license number, you will have 90 days to register and activate your license. Failure to register will result in penalty fees, and after one year, license cancellation.

 

Please note: You must complete initial registration and pay the fee within 90 days of the date your license was issued to avoid penalty fees. If you have not registered your license within 90 days of your license issue date, a penalty fee equal to ½ the registration fee will be assessed. If you have not registered your license within 120 days of your license issue date, the penalty fee will increase to a fee equal to a full registration fee. If you do not register within 12 months of the date of issuance of your license, your license will be considered canceled. 

 

The Board assigns the first registration period. These initial permits will be either 12 or 24 months in length. First time registration fees are prorated accordingly. Subsequent registrations will be two years in length and will be due on the same day each time. Even number licenses expire in even number years and odd number licenses expire in odd number years.

 

The initial registration fee includes an $80 Senate Bill 104 (2003) fee per registration, the $11.48 Prescription Monitoring Program (PMP) fee, and a $5 Office of Patient Protection fee for the first year, with an additional $1 charged for any subsequent year. These fees are required by statute and cannot be pro-rated. The remaining amount is required by the TMB and prorated for each permit length.

The biennial registration fee includes an $80 Senate Bill 104 (2003) fee, the $11.48 Prescription Monitoring Program (PMP) fee, $21.00 for the National Practitioner Data Bank (NPDB), $7.00 for the Physician Health Program (PHP), and a $2 Office of Patient Protection fee. These fees are required by statute. The remaining amount is the fee required by the TMB.

Physician licenses expire at one of 4 times during the year; February 28th, May 31st, August 31st, or November 30th. These registration periods are assigned by the TMB at the time of licensure, and the fees are pro-rated appropriately for the initial registration. Even number licenses expire in even number years and odd number licenses expire in odd number years.

 

After the initial registration, we are not able to offer the option of a shorter or annual registration.

Initial Registration

Subsequent Renewal of Registration

 

There is a 30 day grace period for the expiration date of a physician license. Once the 30 day grace period past the expiration date has occurred, a penalty fee equal to one half of the registration fee will be added. After 90 days, the penalty fee will increase to equal a full registration fee. If a license has been expired for one year or longer it is automatically cancelled.

Online registration:

 

No financial information is seen, processed, or stored by the Texas Medical Board.

 

The payment portion of the online registration system is handled by Texas.gov, the official website of Texas. The price of this service includes funds that support the ongoing operations and enhancements of Texas.gov, which is provided by a third party in partnership with the State. Texas.gov will remit the amount paid to the Texas Medical Board on your behalf.

 

Please note that as of 9/1/2015 Texas.gov will charge a different amount depending on the type of online payment service used.

Non-online (paper) registration:

 

An additional processing fee of $50 will be charged to those licensees who choose to register through a paper form, when an online option is available. If an online option is not available (ex: license is delinquent or suspended at the time of registration) the additional processing fee will not be required.

The sponsoring physician attestation form is to be completed by the registered Sponsoring Physician/Physicians for the Physician Graduate license holder.  This form is required to be completed and submitted to the Board, prior to completion of the subsequent renewal, to confirm that the physician maintains an active relationship with sponsoring physician previously approved by the Board. 

 

This form will be included with a hard copy renewal reminder notice 60-90 days prior to the physician graduate expiration date.

The Texas Medical Board uses electronic permits for a more paperless experience and print on demand for licensees. Once your registration/renewal is complete, please allow 2 business days for processing. After that time, you will be able to log into your MyTMB account and view, save or print a copy of your active license.

 

Please note that primary source verification regarding licensure status is always available to anyone, including licensees’ employers, via the TMB website through ‘Look up a License’.

No.  Under current Board rules, a physician graduate may not have more than one supervising practice agreement, and their practice is limited to the confines of the location documented in the supervising practice agreement submitted to the Board by the sponsoring physician.

At this time Physician Graduate license holders must meet the same continuing education requirements as full physician license holders.

No.  Under current Board rules, a physician graduate license holder is not authorized to delegate to or supervise anyone.
