TMB Suspends Sugar Land Physician (Jaffer)

On December 30, 2025, a disciplinary panel of the Texas Medical Board temporarily suspended, without notice, the Texas medical license of Azul Shiraz Jaffer, M.D. (Lic. No. N2128), after determining his continuation in the practice of medicine poses a continuing threat to public welfare. The suspension was effective immediately.

 

The Board panel found that Dr. Jaffer habitually used drugs or intoxicating liquors to the extent that it interferes with his ability to practice medicine safely.

 

TMB Suspends Addison Physician (Hamlin)

On December 22, 2025, a disciplinary panel of the Texas Medical Board temporarily suspended, without notice, the Texas medical license of Denise Renee Hamlin, M.D. (Lic. No. M6172), after determining her continuation in the practice of medicine poses a continuing threat to public welfare. The suspension was effective immediately.

 

The Board panel found that Dr. Hamlin habitually used drugs or intoxicating liquors to the extent that it interferes with her ability to practice medicine safely.

 

TMB Suspends Austin Physician (Molina)

On December 22, 2025, a disciplinary panel of the Texas Medical Board temporarily suspended, without notice, the Texas medical license of Oscar Aaron Molina, M.D. (Lic. No. T1471), after determining his continuation in the practice of medicine poses a continuing threat to public welfare. The suspension was effective immediately.

 

The Board panel found that Dr. Molina habitually used drugs or intoxicating liquors to the extent that it interferes with his ability to practice medicine safely.

 

TMB Disciplines 13 Physicians at December Meeting, Adopts Rule Changes

At its December 12, 2025, meeting, the Texas Medical Board disciplined 13 licensed physicians.

 

The disciplinary actions included: one related to impairment, three orders related to peer review or other states’ actions, four orders related to standard of care, one order related to unprofessional conduct, one voluntary surrender, one Mediated Settlement Agreed Order, and two Final Orders.

Texas Physician Assistant Board Suspends Texas City Physician Assistant (Seiter)

On December 10, 2025, a disciplinary panel of the Texas Physician Assistant Board temporarily suspended, without notice, the Texas Physician Assistant license of Stephen Lee Seiter, P.A. (Lic. No. PA13946), after determining his continuation in the practice as a physician assistant poses a continuing threat to public welfare after determining his continuation in the practice of medicine poses a continuing threat to public welfare. The suspension was effective immediately.

 

David Lindzey, M.D.

David Lindzey, M.D., of Spicewood, is a physician at Ascension Physician Group. He is a member of the Texas Medical Association and former board member for the Texas Hospital Association and Volunteer Hospitals of America. Lindzey received a Bachelor of Arts in Biology and a Bachelor of Science in Medical Technology from The University of Texas at Austin and a Doctor of Medicine from TTU Medical Center.

Sharon Carr

Sharon Carr, of Canadian, is retired. She previously served as the director of the Hemphill County Juvenile Bootcamp. She is vice president of the board for Hemphill County Hospital and a volunteer with Meals on Wheels. Carr received a Bachelor of Science in Sociology from Texas A&M University (TAMU).

TMB Suspends San Antonio Physician (Perez)

On November 25, 2025, a disciplinary panel of the Texas Medical Board temporarily suspended, without notice, the Texas medical license of Gabriel Moses Perez, M.D. (Lic. No. M8247), after determining his continuation in the practice of medicine poses a continuing threat to public welfare. The suspension was effective immediately.

 

The Board panel found that on November 21, 2025, Dr. Perez was arrested for Aggravated Sexual Assault of a Child, a first-degree felony.

 

TMB Disciplines 21 Physicians at October Meeting

At its October 17, 2025, meeting, the Texas Medical Board disciplined 21 licensed physicians.

 

The disciplinary actions included: seven orders related to standard of care, four voluntary surrenders, four orders related to peer review or other states’ actions, two orders related to unprofessional conduct, two revocations, one order related to impairment, and one Final Order.
